HOW TO JOIN
Simply call or visit our Family Center in either Mobile or Huntsville.
Once you have joined, you will receive your Member Agreement plus two family ID cards. Then just show your ID card and you’ll save money on most of our dental services.
After the first six months, you will pay only $60 per year to continue membership for your entire family.
We accept Visa, MasterCard, cash, or money orders.
The Rules
Primary Member must be a new patient 21 or older.
Plan benefits also include spouse and all legal dependents.

This plan provides discounts at certain dental providers for dental services. This plan does not make payments directly to the providers of dental services. The plan member is obligated to pay for all dental services but will receive a discount from those dental providers who have contracted with the discount plan organization. This discount card program contains a 30 day cancellation period. Member shall receive a full refund of membership fees if membership is cancelled within the first 30 days after the effective date. The range of discounts for dental services provided under the plan will vary depending on the type of dental service received. The discount dental card program makes available, before purchase and upon request, a list of program providers, including the name, city, state, and specialty of each program provider located in the cardholder’s service area. The program is offered by Harbor Discount Plan LLC.
